Alpine Special Treatment Center can bridge the gap between acute care and independent living at a much reduced rate. A referring professional, agency or any adult may refer or request information regarding admission and treatment.

Alpine Special Treatment Center admissions staff are here to assist you in this process. All inquires are confidential and will be handled with respect and compassion. The admission staff understands the special needs of individuals with mental illness and the effects on families and loved ones. We are here to simplify this process and reduce any stress, concerns, or anxiety.

Our admissions staff is happy to work with you on your private insurance benefits coverage however it is the responsibility of the individual to assume responsibility of any co-payments and services not covered.

ASTC recognizes the rights of all individuals and believes that both the individual and family be treated with respect, dignity, and confidentiality. 

Admission Criteria

Alpine Special Treatment Center specializes in adults ages 18 years and older with chronic and persistent mental illness.

All individuals must have a psychiatric diagnosis by a doctoral level clinician.

All individuals referred for treatment must be pre-approved by the admission staff and meet specific guidelines determined by Title 9.

All individuals must be ambulatory, if in a wheelchair the individual must be able ambulate with minimal assistance in the event of an emergency

All individuals must be medically stable; we are unable to accept individuals on oxygen 

Referring agencies please fax the following information to the admissions staff for pre-approval:

  • Face Sheet
  • Name and number of clients payee, if applicable
  • Psychiatric evaluation
  • History and Physical, if physical is >30 days a physicians note stating the individual “has had no changes since last H&P” will be required
  • Conservatorship papers, if applicable
  • Copy of the individuals insurance card (front & back)
  • Any available lab work
  • If the individual is taking routine Clozaril a recent CBC lab (<7 days) must be sent
  • PPD or chest X-ray
  • Current medication list
  • Copy of the physicians referral to Alpine Special Treatment Center
  • 7 days of progress notes (Mental Health Worker notes)
  • Any recent social worker notes
  • Telephone order form the psychiatrist or medical doctor stating the client is “Medically stable to transfer to Alpine Special Treatment Center”
  • If the individual has private insurance, please include the name and telephone number to the Insurance Representative who has given prior pre-approval
  • If the client is a private pay client, our admissions staff will fax you a “Financial Responsibility Form” which must be signed by the responsible party and faxed back to the admissions staff prior to actual admission
